IATA P650 Triple Packaging for Biological Specimens: What It Requires and Why It Matters

IATA Packing Instruction 650 (P650) defines the triple-packaging system mandatory for all Category B biological substances and patient specimens transported by air from India. Non-compliant packaging results in cargo rejection at the airline acceptance desk  and potential regulatory penalty.

Primary receptacle

Watertight, leak-proof container holding the specimen, e.g., sealed tube or cryovial. Must withstand internal pressure differential during flight without leaking.

Secondary packaging

Watertight, sealed outer layer surrounding the primary receptacle. Must contain absorbent material sufficient to absorb the entire contents of the primary receptacle if it leaks.

Outer packaging

Rigid outer box (cardboard or hard-sided) meeting IATA strength requirements, labelled with the UN3373 biohazard diamond, specimen data form, and sender/receiver details on the exterior. For dry ice (UN1845), a quantity declaration was added here.

Packaging selection and preparation

Documentation preparation

Acceptance and airside handling

Cargo is presented to the airline’s DG acceptance desk with full documentation. Clearfast staff manage any airline queries, physical inspection coordination, and re-icing if required before departure.
